#1 > Inguinal Hernia

An inguinal hernia is bump in you groin.
It can cause pain and discomfort and also causes a bulge through the skin.
Sometimes the bulge it is not easy to detect and only an experienced surgeon like Dr. Garcia can make the diagnosis with a dynamic ultrasound.

Robotic Inguinal Hernia Repair

Robotic surgery provides a number of advantages over traditional open surgery and standard laparoscopic procedures.
Garcia goes behind the muscles and retrieves the hernia that went through the muscle. He then places an ultralight mesh to cover the hernia defect or defects.
Eliminates almost any mesh related long term issues.
It is the most complete technique available.
It prevents nerve entrapment seen in open surgery.
It allows for a speedy recovery.
Robotic surgery allows Dr. Garcia to repair both direct and indirect hernia and bilateral hernia at the same time.
The robotic surgery system, which is controlled by the surgeon at all times, uses tiny instruments and 3-D cameras to precisely repair the affected area.

Open Air Inguinal Hernia Repair

It is currently used by Dr. Garcia when the hernia is extremely large.
When patient’s medical conditions contraindicate laparoscopy surgery.
The open technique or laparoscopic technique uses ultrapro-plug system or absorbable mesh.
The results are excellent in Dr Garcia’s hands.

The Laparoscopic Technique

This technique is slowly being abandoned due to the better robotic visualization.
Laparoscopic repair is done in a virtual space which is created by a balloon in the preperitoneal space.
It is not as accurate a dissection as a robotic dissection.
The benefits show to be as good as robotic in large studies.
The laparoscopic technique provides faster recovery, but it is limited for recurrent hernia, bilateral hernia and athletes and workers that need an intimidate return to activities.

#2 > Umbilical Hernia

An Umbilical Hernia occurs when intestine, fat, or fluid pushes through a weak spot in the belly. This causes a bulge near the belly button, or navel. The umbilicus is prone to muscle weakness since it’s where the greatest stress is placed on the muscles when we do anything physical. In children these hernia often go away on their own, but in adults these hernias never get better and usually get bigger over time.

An umbilical hernia occurs when intestine, fat, or fluid pushes through a weak spot in the belly. This causes a bulge near the belly button, or navel.
It can cause pain and obstruction.
It generally needs to be repaired.
Garcia uses a new state of the art robotic technique.
He opens the space behind the rectus muscle and closes the defect with a baseball stitch using the robot.

Then he places thin small mesh behind the muscle (Sublay).

Garcia then closes the peritoneum and prevents the mesh from creating any problems.

Complex hernias are defined as hernias of the abdomen that have recurred despite multiple repairs, that are associated with infections, or have resulted in bowel perforations or fistulas.
